This form must be completed no less than one hour before you enter the Place des Arts facility, or any authorized satellite site hosting PdA programs (“Facilities”), to perform work.

The health and well–being of our Place des Arts (PdA) community is our priority. We are taking measures to keep PdA a safe environment for all. 

Therefore, PdA requires that all its employees and self-employed contractors (“Workers”) complete a COVID-19 Worker Health Declaration (WHD) webform prior to each date upon which they enter the PdA building, or any authorized satellite site hosting PdA programs (“Facilities”), to perform work. 

The WHD is a screening questionnaire used to identify Workers: 

• who may be experiencing COVID–19 symptoms, as characterized most recently by federal and provincial public health officials; 
• who have knowingly come into close contact with anyone who has tested positive for COVID-19 or who has been ordered to self-isolate (including a member of their household); and/or  
• who have travelled outside of Canada. 

Upon each entry to the Facilities to perform work, the Worker must complete the WHD webform truthfully and to the best of their knowledge. The WHD webform must be completed and submitted no less than one hour prior to the Worker’s scheduled start time. 

A Worker who refuses to complete the WHD will not be permitted to enter the Facilities. In these instances, that Worker will be notified of any relevant policies and procedures. 

Upon entering the Facilities, each Worker must follow physical distancing and personal hygiene protocols. These protocols include handwashing, disinfecting keys, phones, and other personal items, avoiding touching one’s face and coughing and/or sneezing into a disposable tissue or one’s elbow crease.
